Two (2) copies of the following in a long brown envelope:
2. NCAE/MATB/YP4SC profiling
3. High School Card (Form 138) or Secondary Permanent Record (Form 137A)
4. Birth Certificate
5. Medical Certificate
6. Certificate of Good Moral Character
7. Barangay Clearance
8. 2 pcs. 1X1 and 2 pcs. 2×2 colored picture with white background, with collar
9. Computer literate
